After paying for the tests, please fill out the following forms and email to daltrainerf @ yahoo.com
Thyroid Testing
Access the OFA applications by clicking here
Once you access the page, click on Thyroid, Single Animal and then click the orange button “Fill out Form.”
The following form will be displayed. Do not fill out anything in the data field Vet Lookup or any fields after Vet Lookup. Click on orange button “Download PDF Form(s)” to save to your Download Folder on your computer. Email this form to Kelly Flannigan
CHIC DNA Testing
Access the CHIC DNA application/Health History Form by clicking here
This will bring up a 2-page fillable form. You need to fill out the both pages completely. Save to your computer and email to Kelly Flannigan
